Workplace Giving

Your company likely uses one of two ways to secure your additional matching gift to NAMI Indiana:

• A matching gift form. Obtain the form from your company, fill out the “donor” section and mail it to NAMI Indiana. NAMI Indiana will complete the “charity” section of the form and mail it back to your company. This will confirm your gift and request their matching gift.

• A receipt of your gift. Make your donation to NAMI Indiana through the mail, phone, or online. Upon receipt of your gift, NAMI Indiana will send acknowledgement of your contribution. Bring this document to your company, who will distribute their matching gift to NAMI Indiana.
